- Insurance Claim Requirements: All hospital payment receipts must be attached.
- Maximum age: 55 years.
- Period of cover: 365 days.
- Risk Location: Worldwide.
- Deductible: Nil.
- Not including Natural Death.
- The inpatient receipt can be a photocopy that has been legalized/authenticated by the hospital.
- Disbursement claim method is reimbursement by including all necessary documents to the insurer.
- Terms of membership: Insured age max 55 years when purchasing a Policy.
- For hospitalization claims, there will be 14 days waiting period after the effective date stated in the e-policy.
Easy Steps to Secure Life Insurance
1. Procedures to register as Policyholder/Insured
- Every request must fill in and sign the Closure Request Letter (SPPA).
- SPAA should attach several information of participant including:
- Insured Name
- Place & Date of Birth
- Job Status
- ID Number
- Scan copy/photo passport/KTP/KITAS/KITAP/KIMS
- And several other information
2. Risk Survey
- The insurer may request to conduct a risk survey at the accident/hospital location.
3. Insurance Claim Settlement Procedures
- Immediately report to the Insurer no later than 3 x 24 working hours after the incident.
- Fill in the claim signed by Insured.
- Attach supporting documents:
- For treatment at a Hospital: receipt/treatment details (original or copy legalized by the Hospital) and all other supporting documents if any (invoice of medicine, invoice of lab/rontgen, summary of medicine, result of lab/rontgen)
- For death in the form of: Certificate from the sub- district or police or doctor/hospital
- The maximum claim file submission deadline is 30 working days from the date of the incident/loss. Claims are considered expired if within 30 working days the policyholder or family does not complete the claim requirement documents.
